Under supervision of the Promesa Community Residence Assistant Director, the Community Residence LPN is responsible for overseeing the medication administration of the community residence program.  This individual must demonstrate the knowledge and skills needed to provide appropriate care based on the assigned unit. This position will adhere to all policies, procedures and regulations set forth by NYSOMH to ensure patient safety and compliance. The requirements listed below represent the knowledge, skill, and/or ability required. KEY ESSENTIAL FUNCTIONS:                                                                                                                           
  • Oversees the medication administration in the Community Residences
  • Ensures Medication Administration Records (MARS) are up-to-date and in compliance.
  • Meets with residents regarding medication to ensure understanding of prescribed medication
  • Educates Community Residence Aide about the medications prescribed to the residents of the facility
  • Oversees the overall functionality of the medication room.
  • Communicates as indicated with prescribing providers.
  • Maintains accurate documentation of all nursing activities.
  • Communicates with providers as indicated for hospitalized residents to ensure seamless transition back to residence upon discharge.
  • As a part of the initial evaluation and the 90-day reassessment, reviews client"s medical history including medication.
  • Carries out established policies and procedures for medication and treatment administration
  • Ensures all residents are stable medically.
  • 3+ year"s previous relevant experience as a License Practical Nurse preferably in Mental or Behavioral Health environment.
  • Associate"s Degree or an equivalent related field required
  • New York State Licensed Practical Nursing license required.
  • New York State CPR certification preferred.
  • Must be able to multi task with strong organizational skills.

    • Ability to multitask and to work and function under pressure
PHYSICAL DEMANDS & WORKING ENVIRONMENT:                                                                                     
  • Shift work required
  • All work is completed on campus
  • Follows clearly defined policies and procedures
  • May require work outside of standard work hours or on weekends
  • Subject to infectious waste exposure (typical of most patient care/housekeeping positions)
  • Light work (lifting approximately 20 pounds maximum with frequent lifting and/or carrying of objects up to 10 pounds.  May involve significant amounts of walking, standing, pushing, or pulling)
  • Sometimes there will be bending, climbing, kneeling, reaching, lifting and extended sitting
  • Extended standing (requires standing without being able to sit for long periods of time)
  • Frequent visual inspection (such as reading calibration marks on syringe or examining small circuitry)
  • Extended walking (requires walking without being able to stop or rest for long periods of time)
