POSITION OVERVIEW:                                                                                                                        
The Medical Records clerk will effectively coordinate and monitor all Primary Care's patient records. The Medical Records Clerk will gather appropriate record folders and contents; assigning and recording new record numbers; lifting existing record numbers; inputting and recording locations to computer; delivering records. Supervision, maintains patient files, statistic and ensure payment for agency copies prior to forwarding request(s). Will work closely with the networks Legal department.

KEY ESSENTIAL FUNCTIONS:                                                                                                            
  • Gathers patient information by collecting demographic information from a variety of sources, interacting with registration/ front desk areas and providers offices
  • Maintains master patient in completing assigned portion of daily audit trail; corrects and communicates.
  • Initiates the medical record by creating and processing the patient care record folder.
  • Maintains record availability by processing charts into the department; using chart mark-off procedures; facilitating chart location activitie
  • Maintains continuity of work operations by communicating actions,
irregularities, and continuing needs
  • Maintains patient confidence by keeping patient records information confidential
  • Serves and protects the agency by adhering to professional standards, policies and procedures, federal, state, and local requirements, and HIPPA standard
Enhances medical records reputation by accepting ownership for accomplishing new requests: exploring, opportunities to add value to job accomplishments.
  • Clerk will adhere to HIPPA
  • Responds to requests for medical records; processes letters and reports; answers and directs telephone calls
  • Retrieves patient schedules, from computer system when needed
  • May photocopy records and documents for billing and/or legal services; sends and receives
  • Keeps Supervisor informed with any problems or issues, monitors supplies needed; performs other duties as assigned
