Position Summary:
Our Social Services Department is seeking a Social Worker Level III for its Integrated Care team. This position will operate out of our CitiCares Community Health Center in Central Harlem. As an integral part of our disciplinary team, the Social Worker collaborates with medical providers to administer comprehensive services to patients that address psycho-social needs in family medicine.
Minimum Qualifications:
- Masters in Social Work is required.
- NYS Licensed Masters Social Worker is required.
- 5 years of direct LMSW experience, including work with patients who present with mental health and/or substance abuse.
- Crisis intervention experience.
- Microsoft Office Suite.
Key Duties:
In addition the duties described above in the position summary, additional duties to include:
- Provides social work support and resources for patients in need of food, shelter, or other social indicators of health.
- Assists providers in addressing needs of patients with complex mental health and/or substance use barriers.
- Provides crisis intervention services to patients.
- Provides support to patients in the MAT Program.
- Conducts psychosocial and assessments to develop plans of care.
- Creates linkages for patients with services including psychotherapy, detox, and other specialized programs.
Starting Compensation:
Minimum $75,000 per year; Maximum $75,000 per year
Work Environment Type, Location, and Schedule:
Onsite; Harlem, NY; Monday through Friday 8am-5pm
