Position Summary:
We are hiring a Medical Assistant/Patient Navigator who will assist provider making medical home visits to home bound patients including taking chief complaints, taking vital signs, and following provider orders for generating specialty and support referrals.
Minimum Qualifications:
- High School Diploma required
- Medical Assistant Certificate or completion of an accredited Medical Assistant program
- 2-5 years of related experience in the healthcare sector
- Home visits with the MHVP team 1-3 days a week.
- Bilingual English/Spanish is required
- Proficiency with computers and software
- Strong communication skills
- Ability to multi-task and thrive in a high-volume environment
- Kindness, compassion, and patient-centered work ethic
Key Duties:
- Assists with Medical Home visit pre-visit planning, calling patient, conducting medical intake, etc.
- Prepares for Medical Home Visits ensuring PPE, equipment, transportation, and patient information forms.
- Takes patients temperature, BP, weight, respiration, & personal information and records it in the patient's medical record.
- Assures that all patients are properly put into the computer, and that the patient rights, advance directives are. communicated to the patient.
- Assist in the tracking of patient services including recalling of patients based on diagnostics test results
Starting Compensation: Minimum: $21.42/hour ; Maximum: $21.42/hour

Work Environment Type, Location, and Schedule:
Candidates must possess availability to work onsite based out of our Bronx Community Health Center, El Nuevo San Juan and be comfortable with conducting home visits. Schedule will be Monday - Friday 8:00am to 5:00pm.
