Position Summary:
Under the supervision of the Screening Supervisor, the Screening Specialist will address behavioral health and social service referrals made by providers in the social work referrals box. The Screening Specialist will assess pts needs and provides appropriate interventions including internal/external behavioral health and social services referrals and escalates crisis intervention and complex cases to Licensed Social Workers. The Screening Specialist also provides immediate concrete services to patients who are referred through the 'needs social work box' including but not limited to housing, food insecurity, etc.
This Screening Specialist will work remotely, Monday - Friday 8am-5pm with 1-2 late nights per week. While working remotely, the Screening Specialist will need to be flexible to work onsite on a as needed basis, attending bi-weekly meetings and attend in person trainings when needed.
Compensation: $50,560 based on 40 hours per week.
Minimum Qualifications:
- Bachelor's degree in Psychology, Social Work, or related field required.
- 2 or more years of experience with case management.
- Knowledge of programs and resources.
- Ability to multi-task, and to manage several requests via phone or computers.
- Excellent communication skills.
- Dedication to supporting patients in our communities.
- Bilingual Spanish/English is required.
Key Duties:
- Reviews referral box and conducts a thorough review of pts electronic health record for potential patient's needs.
- Conduct BH referral providing internal or external appointments.
- Assists with positive score on GAD-7, AUDIT C and/or PHQ9, (Review and make referrals)
- Assist patients confronting food insecurity.
- Assist patients confronting housing challenges.
- Assist in providing information to parents and assisting with IEP educational issues.
- Assist with scheduling medical and other support appointments.
- Provides direct concrete services as needed and assist patients with other (non-clinical) needs as presented.
- Contacts pts to verify needs, including those not mentioned in referral.
- Addresses all patient needs through internal/external referrals.
- Escalates high risk, complex pts to the Social Work team including cases in crisis.
- Documents all contacts and interventions in eCW
