Position Summary:
The Healthy Beginnings Program Coordinator will coordinate all aspects of the program including but not limited to identification and enrollment of OB patients seen at ENSJ who are deemed high risk based on psycho-social screening and other risk factors. The coordinator is the administrative lead of the Healthy Beginnings Team designed to manage enrolled patients from prenatal care through the infants' first year of life supporting full engagement in prenatal and pediatric care. The coordinator leads team meetings, directly supervises the program social workers and care associates and participates in the collection and analysis of program data, meeting or exceeding all grant deliverables.
This role is hybrid with 4 days onsite and 1 day remote.
Minimum Qualifications:
- Masters Degree in Social Work is required.
- Bilingual Spanish/English is strongly preferred.
- 2-5 years' experience with program coordination
- Previous experience supporting maternal/child health is a plus!
- Excellent interpersonal skills including over the phone, in person, and e-mails.
- Computer skills including Microsoft Office, Data-Driven software programs, electronic records, etc. are essential.
- Ability to review, track, and report out on data- including analysis of complex excel data
- Thrives in fieldwork driven role with comfortability with facilitating home visits for the program.
Key Duties:
- Coordinate all aspects of the Healthy Beginnings program ensuring UHP meets or exceeds grant deliverables under administrative and clinical leadership.
- Create and implement a process for identification and enrollment of OB patients seen at ENSJ who are deemed high risk based on psycho-social screening and other risk factors.
- Act as administrative lead of the Healthy Beginnings team designed to manage enrolled patients from prenatal care through the infants first year of life supporting full engagement in prenatal and pediatric care.
- Plan and lead team meetings and case reviews to ensure all enrolled patients have a discrete care plan that meets their individual needs.
- Supervise the program social worker ensuring that they conduct assessments, depression and social needs screenings, counseling, complex care coordination and provide support to enrolled patients.
- Supervise care associates ensuring they track assigned patients and support them in care coordination, continuity of care, care planning and treatment adherence.
- Ensure the tracking of the transition of patients to the delivery site and create system to bring baby back for well child visits.
- Where possible, create a system for post-delivery visits to the hospital to ensure continuity of care for mother and children.
- Lead the team in implementing program engagement events such as baby showers, art integration, mommy and me classes, healthy cooking classes, healthy post weight pregnancy, patient listening sessions, etc.
- Provide enhanced services with the pediatric team to ensure infants attendance at well child visits and adherence to immunizations.
- Ensure the integration of complimentary services including but not limited to the Nurse Family Partnership, UHP's MICHC and PICHC programs, WIC etc.
- Support all aspects of data collection, QI/QA and data collection, analysis and program evaluation.
Work Environment Type, Location, and Schedule:
Hybrid, Bronx, NY, Monday - Friday 8am to 5pm and/or 9am-6pm.
Compensation:
Minimum: $75,000 annually; Maximum: $80,000 annually
