Position Summary:
The Scheduling Coordinator coordinates, administers, and promotes the success of the partnership between Urban Health Plan (UHP) and CenterLight PACE (CL). This role coordinates the transition of patients referred through CL to UHP for specialty services and maintains the administrative and communication needs centered around the bi-directional nature for both patients referred through CL for specialty services AND facilitating the linkage of current UHP patients who may be eligible to enroll into CL's PACE program. The Patient Registration Coordinator oversees the successful creation of the referred patient's chart in ECW based on the faxed account referred through CL's EMR, obtaining necessary consent and documentation, schedules the appointment with the patient, informs CL of appointment, conducts prior authorizations, reviews ECW imports and validates patient records, and maintains ongoing and professional communication as a liaison to all parties (Patient, Provider, UHP, CL Rep, etc.)
This is an onsite role, Monday through Friday 8am-5pm.
Key Duties:
- Manages all incoming referrals for new CL participants accessing UHP specialty care including opening charts and obtaining required documentation per CL and UHP protocols. This includes ensuring consent for treatment is in the chart prior to CL participants appointment.
- Schedules/reschedules appointments for new UHP patients referred to specialty services through CL and ensures that when transportation has been arranged by CL process, when applicable.
- Communicate any changes or updates to CL regarding appointments when canceled, rescheduled, or moved.
- Following visit with UHP specialist, obtains required prior authorizations for referrals, diagnostic testing etc., following both CL and UHP protocols including STAT procedures.
- Provides support for follow-up appointments working with CL's counterpart for established CL participants accessing UHP specialty care.
- Utilizes all approved technological platforms that support protocols and coordination of care for CL patients accessing UHP specialty services, including the bidirectional exchange of information for UHP patients enrolled in PACE services per UHP and CL protocols.
- Participates and presents in joint UHP-CL interdisciplinary team meetings, including status updates, progress, and identification of process improvement measures.
- Ensures that all required documents are in place for UHP patients that enroll into CL PACE program.
- Communicates with UHP finance department to ensure patients' insurance record accurately reflects CL PACE.
- Once a UHP patient is established as a participant in the CL PACE program, ensures completion of required follow ups, authorizations for referrals, diagnostic testing, etc. following both CL and UHP protocols including STAT procedures.
Minimum Qualifications:
- High School Diploma is required
- Associate's degree or Medical Assistant certification is a plus.
- A minimum of 3-5 years' experience carrying out the following functions is required: Referrals processing, patient services/registration, patient intake, and prior authorizations.
- Bi-Lingual in English and Spanish is required.
- 3 years of successful utilization of EMR is required.
- Ability to communicate with high level of professionalism, both verbally and in writing.
- Ability to understand data, report out on data, and present to various stakeholders.
- Proficiency with Microsoft Office including Outlook, Word, Excel, and PowerPoint
- Knowledge of ECW is strongly preferred.
Starting Compensation: Minimum: $50,000 per annum; Maximum: $50,000 per annum.
