Position Summary:
As the head of Urban Health Plan's Nutrition Department, the Senior Director of Nutrition Services will be responsible for clinical nutrition services, community nutrition services and food insecurity programming throughout UHPs network of health centers and the communities we serve. The Senior Director has direct reports that including CDN/RDN Nutritionists, Nutrition Counselors (Bachelors in Nutrition), and administrative support staff. The incumbent will also support managing multiple nutrition grant and non-grant programs to address patient and community needs, manage our Demo Cooking Kitchen program and work towards ensuring that our communities can receive comprehensive support with addressing food insecurity, preventing chronic illness, and improving chronic illness management through nutrition services.
Key Duties:
Clinical Nutrition Responsibilities
- Plans, directs and coordinates activities of the Nutrition Department to ensure that nutrition services are provided to all population groups. Directly supervises all nutrition program staff.
- Assures that Nutrition standards of care are maintained throughout UHP's network of health care, monitors monthly department dashboard.
- Establishes department regulations and procedures in conformance with administrative policies and develops standards for the organization, and supervision of nutrition services.
- Assesses the quality and quantity of nutrition clinical services in accordance with established policies and procedures.
- Provides and updates Urban Health Plan's Nutrition Manual so it coincides with UHPs medical nutrition procedures. The manual should be current to reflect advances in the science of nutrition and the medical nutrition protocols. Should be up to date on the medical nutrition of acute and chronic diseases.
- Support of team based interdisciplinary care and PI teams, and management of outcomes through quality dashboard.
- Assesses and/or develops nutrition educational programs and materials based on patient needs assessments
- Provides nutrition education/counseling in the prevention and/or management of disease to individuals and families or significant others. Instruct patients and their families in nutritional principles, dietary plans, and food selection and preparation.
Community Nutrition Responsibilities:
- Lead, manage and supervise nutrition and food insecurity programs organization wide including grant and non-grant programs, ensuring deliverables are met or exceeded and reporting is complete and timely. Programs include but are not limited to the emergency food pantry, food distributions, farmers market program, health bucks, produce prescription program. Acts as a Liaison with Food Resource Coordinator in the coordination of external food insecurity programming.
- Manages grants as assigned ensuring data is collected, deliverables are met on time, all reporting/evaluation is completed.
- Plan, manage and track demo kitchen programming and activities.
- Directly supervises staff assigned to special grants and demo kitchen initiatives and programs.
- Plan and oversee various annual nutrition activities in and out of the health center setting. This includes nutrition workshops, health fair events and food demonstrations.
- Participate in community nutrition & organizational health program's/special events.
- Develop and manage key partnerships that forward the organization's nutrition department goals.
- Represents Urban Health Plan with outside agencies advancing mutual community
Minimum Qualifications:
- Masters Degree in Nutrition and NYS Licensed CDN is required.
- 5+ years of experience providing direct clinical nutrition services to patients.
- 5 years of experience with managing nutrition programs, nutrition grants, or other related management and oversight of clinical nutritionists.
- Bilingual English/Spanish strongly preferred.
- Proficiency with electronic health records, Microsoft Office Suite, and other related software.
- Leadership expertise and ability to effectively communicate with various stakeholders.
Starting Compensation: Minimum: $130,000 per annum; Maximum: $145,000 per annum.
