Position Summary:
The Virtual Care Center Clinical Pharmacist is a full-time clinician who will work on conducting medication reconciliation and transition of care medication management for patients recently seen in the ER or hospital. The clinical pharmacist will provide patient counseling, drug information services, patient and staff education. They function as clinical support for all layers of the clinical teams within UHP.
This position is hybrid with 4 days remote and 1 day onsite in the Bronx.
Minimum Qualifications:
  • Doctor of Pharmacy Degree required from ACPE accredited college. 
  • Experience working with patients who suffer chronic health conditions.
  • NYS Licensed Pharmacist
  • Must possess Postgraduate Year 1 (PGY1) and/or Postgraduate Year 2 (PGY2) in Ambulatory Care Residency
  • Bilingual English//Spanish highly desired.
Key Duties:
  • History and assessment of patients by interviewing patients and caregivers via med reconciliation and allergies.
  • Updating medication lists after emergency room or hospitalizations through reconciliation process utilizing patient accounts, commercial pharmacies, hospital discharge information, and other patient information platforms
  • Supports team-based approach to assure highest delivery of care outcomes.
  • Function as a clinical resource for the VCC RN and clinicians.
  • Documents patient care services by charting in patient medical record.
  • Assists in the development of patient management plans, records progress notes, and assists in provision of continuity of care.
  • Instructs and counsels patients by describing therapeutic regimen plans and answering any questions patients have.
  • Provides continuity of care by developing and implementing patient management plans.
  • Participate in other clinical innovative therapeutic programs such as RPM and CGM by doing analysis of CGM device reading and determining which patients need therapeutic interventions.
  • Work as the clinical support for the prior authorization process when clinical expertise is needed
  • Provide patient education and counseling
  • Participate in educational endeavors of UHP and engage with learners and staff at all levels
  • Assist the risk management team with pharmacologic expertise in assessing unexpected events
 
Starting Compensation: 
Minimum $120,000 per year; Maximum $125,000 per year
 
Work Environment Type, Location, and Schedule:
Hybrid, Bronx, NY, Monday to Friday 8:00 am to 5:00 pm.
