POSITION OVERVIEW:
 The Psychiatric Nurse Practitioner (PNP) is responsible for professional health care services, in accordance with generally accepted standards of medical/nursing practice for individuals experiencing homelessness as part of Promesa's Intensive Mobile Treatment (IMT) program. The PNP will administer psychiatric diagnostic evaluations and prescribe and support the administration of medications, including medication assisted treatment (MAT) for substance use disorder (SUD). The PNP will also be on call for psychiatric emergencies among IMT consumers. The PNP ensures strengths- based, person-centered, and quality care provided in a culturally, linguistically, and meaningful therapeutic environment. KEY ESSENTIAL FUNCTIONS: 
• Active staff member of the Integrated Mobile Treatment team, participation in grant activities, completion of grant trainings, attendance at meetings, service delivery as per health disparities indicators, psychiatric assessments, symptom monitoring, and medication management. 
• Obtain complete medical history and physical data including vitals on patient using evidence-based assessment tools as per organizational policy. 
• Evaluate and interpret patient data to plan and implement appropriate medical interventions and decrease health disparities indicators. 
• Provide general nursing services within scope of license and patient safety goals, including providing psychiatric assessment and treatment, and the evaluation, prescription, and monitoring for psychiatric medication and medication assisted treatment for substance use disorders. 
• Provide psychiatric support in crisis situations, either via telephone or in person. 
• Conduct integrating care by completing medical records and coordinating with other providers. 
• Conduct patient education after assessing patient needs and prior knowledge of medical record. 
• Conduct health-wellness promotion activities to educate patients on best health care practices. • Participate in daily IMT staff and organizational meetings 
• Practice according to the principles of recovery oriented and trauma informed service delivery 

EDUCATION AND EXPERIENCE REQUIRED: 
• Registered Nurse with completion of accredited Nurse Practitioner (NP) program. 
• Three years of experience in ambulatory care and working with people with substance use disorder. 
• Bilingual (English/Spanish); ability to communicate effectively orally and in writing. 
• Experience working with homeless individuals a plus. 
• Excellent time management, organization, motivation, team-oriented, communication, and computer skills, including Microsoft Windows, Excel, PowerPoint, and the internet
